Backend Web Application Engineer

Engineering Remote · US Full-time Founding equity

Role description

The backend engineer maintains and extends the Laravel application behind SynAIpse: EHR integrations, the deterministic coding rule engine, and the data pipelines that feed our machine learning systems.

You work alongside the founders, working across EHR integrations including eClinicalWorks, Epic, and athenahealth and shaping the architecture as we expand to new specialties.

What you will do

  • Extend our Laravel application to support EHR integrations (eClinicalWorks, Epic, athenahealth), extending via HL7v2, FHIR, and Cloud Fax.
  • Iterate on our deterministic coding rule engine, including NCCI edits and payer-specific requirements.
  • Build a self-service onboarding flow so new practices can provision their own integrations without us doing it manually.
  • Build the data labeling interfaces and pipelines that turn coder and user input into training data for our models.
  • Implement services that meet HIPAA, SOC 2, and HITRUST standards.

Qualified applicants have done this

  • 4+ years with Laravel or an equivalent MVC framework.
  • Deployed systems that required 24/7 stability and availability.
  • Strong written and verbal communication — able to translate technical detail for non-technical audiences.
  • Implemented logging and auditing controls for access to and disclosure of Protected Health Information (PHI) under HIPAA.

Exceptionally qualified candidates have done this

  • Direct experience with one or more EHR APIs.
  • Worked with HL7v2 or FHIR resources.

Frontend Web Application Engineer

Role description

The frontend engineer builds the surfaces that make our coding platform feel like a tool serious medical coders want to use. You work primarily with Laravel Livewire and AlpineJS, owning the coder workflow, the surgeon report finalization UI, and the operational dashboards that keep our practice partners on track.

You sit close to back-office staff, physicians, and deployment telemetry, refining interfaces to fit how the work actually happens.

What you will do

  • Design and build Laravel Livewire components and AlpineJS interactions across the coder workflow, the surgeon report finalization UI, and the admin operations surface.
  • Keep every SynAIpse interface responsive to user input within 100 ms.
  • Implement a keyboard-shortcut-driven workflow alongside the standard point-and-click options.
  • Build interfaces that meet WCAG accessibility standards.
  • Write automated UI tests so deployments don't silently break user flows.
  • Work with backend engineers to reduce round-trip requests where possible.

Qualified applicants have done this

  • Four years building production web applications.
  • Strong production experience with Livewire, Alpine.js, or other popular JavaScript libraries like React, Vue, or Svelte.
  • Written automated UI tests against real user flows.

Machine Learning Practitioner

Role description

The ML practitioner runs our coding pipeline in production, builds the evaluation infrastructure that lets us measure and improve accuracy, and owns in-house fine-tuning as our specialty coverage and customer base grow.

What you will do

  • Use the GCP AI ecosystem to fine-tune our in-house revenue cycle management models.
  • Create qualitative and quantitative benchmarks to measure model quality as it evolves.
  • Work with data labelers — medical professionals and billers — who evaluate model output.
  • Train models on feedback from the application as coders accept or reject suggested codes.

Qualified applicants have done this

  • Fine-tuned LLMs using LoRA with PyTorch or JAX.
  • Worked in a major cloud provider's AI ecosystem (GCP, AWS SageMaker, Azure ML) for model deployment and monitoring.

Customer Success Manager

Role description

The Customer Success Manager helps customers onboard their data and get value from the platform. You meet customers shortly after they sign, identify their EMR, and work with the people in charge of billing.

From there, you build the launch timeline that takes them from where they are today to running on SynAIpse, and train physicians and non-clinical staff so they get the most out of the platform.

What you will do

  • Own the customer onboarding journey, including EMR integrations or, where no EMR is in place, walking users through the SynAIpse data onboarding application.
  • Develop written, verbal, and presentation-based training so users adopt SynAIpse safely and effectively.
  • Stay on as the client's ongoing point of contact for support questions.

Qualified applicants have done this

  • 2+ years in a customer-facing role: Customer Support, Customer Success, Account Management, or implementation at a B2B SaaS company.
  • Strong written and verbal communication, with an ability to adjust messaging for audiences of varying technical skill.
  • Project management discipline: keeping tickets up to date across clients so every client is well supported.
  • Comfort using internal troubleshooting tools to help customers resolve technical issues.

As founding COO, you build the operational backbone of Synaipse. Our physician co-founders own product and clinical strategy; you own go-to-market, partnerships, finance, and the systems that let a small team execute beyond its size. You report directly to the CEO.

You're not inheriting processes. The first enterprise contract, the sales playbook, vendor relationships, the hiring rubric — you build them from scratch and own them long-term.

What you will do

  • Go-to-market strategy and execution: identify target customer segments, build the sales pipeline, and close deals with independent practices, surgical groups, and health systems.
  • Strategic partnerships with EHR vendors, payer organizations, medical societies, and healthcare technology platforms.
  • Financial planning and operations: budgeting, forecasting, fundraising support, and unit economics.
  • Operational infrastructure: the processes, tools, and workflows that let a small team execute at enterprise scale.
  • Team building: recruit, hire, and develop the founding team across sales, customer success, and operations.

  • 8+ years in operations, strategy, or business development, with 3+ years in a leadership role at a high-growth startup or healthcare company.
  • Taken a B2B SaaS or healthcare product to market, ideally from early stage through initial revenue milestones.
  • Deep understanding of the healthcare landscape: payer dynamics, practice operations, physician-owned credentialing workflows, or revenue cycle management.
